I confess after reading the article that I don't understand what the term means.

I got a vague ambiguity from the article. Some googling did not help. More to come perhaps?

Mafiosi
This has a little background:
https://en.m.wikipedia.org/wiki/Operation_Aderlass
I think “mafia” is just used generically to indicate a rogue operation ouitside the team structure. Maybe the athletes just sign up and pay independently...or not.... Jes’ good ol-fashioned blood dopin’....

"suspected athletes could be escaping detection because they are transfusing blood in the morning of a race and extracting blood directly following the race."
why directly following/after I wonder...
And came across this:
AICAR/GW1516, which is also known as GW501516 and marketed as Endurobol.
Google it if this is a rabbit hole you'd like to peek into.
