In the top photo, it looks like the rider's saddle is way too low.
After watching the video, he indicated that he has an unusual position because he drops his heels much more than most people.
As for the fore-aft position of the cleats, I agree that they should be behind the metatarsal head, but I have no idea how he's getting them as far back as he recommends. I've been riding Speedplays for over 20 years and the biggest issue I've had with them was the inability to get them back far enough. It makes me wonder where he's actually measuring from, as in the video it looks like he's measuring from the metatarsal

, rather than the

, which is the standard that I've always seen. Measuring from the joint does make sense, as it's more distinct than trying to approximate the center point of a curved bone.
In my experience, the worst type of cleats are the Look style, where the pivot point is way forward on the shoe. It's probably fine if you have the no-float cleats, but if you use their floating cleats, it can create major foot instability. That's the main reason I dumped them in favor of Speedplay.
